Beschrijving Vardenafil HCl Trihydrate (BAY38-9456) is een nieuw type PDE-remmer met een IC50 van respectievelijk 0,7 en 180 nM voor PDE5 en PDE1.

In vitro Vardenafil remt specifiek de hydrolyse van cGMP door PDE5 met een IC50 van 0,7 nM (6,6 nM). Vardenafil versterkt significant de SNP-geïnduceerde relaxatie van menselijk trabeculair glad spierweefsel bij 3 nM (10 nM). Vardenafil versterkt ook significant zowel de ACh-geïnduceerde als de transmuraal elektrische stimulatie-geïnduceerde relaxatie van trabeculair glad spierweefsel. Vardenafil (100 mM) verhoogt de cyclische GMP-niveaus in ratten hippocampus plakjes. Vardenafil, tadalafil en Sildenafil remmen elk competitief de cGMP-hydrolyse door phosphodiesterase-5 (PDE5), waardoor de cGMP-accumulatie en relaxatie van vasculair glad spierweefsel worden bevorderd.
In Vivo Vardenafil versterkt dosisafhankelijk de erectiele responsen op intraveneus toegediend natriumnitroprusside bij konijnen. Vardenafil (3 mg/kg, p.o.) resulteert in een verbeterde objectdiscriminatieprestatie bij ratten. Vardenafil (30 mg/L, p.o.) verhoogt zowel de iNOS- als de prolifererende celnucleaire antigeenexpressie (SM-celreplicatie) bij ratten, met normalisatie van de dynamische infusie-cavernosometrie-daling en de SM/collageenverhouding. Vardenafil induceert een krachtig preconditioning-achtig cardioprotectief effect tegen ischemie/reperfusieletsel door opening van mitochondriale K(ATP)-kanalen in het hart van het konijn. Vardenafil beschermt het ischemische myocardium tegen reperfusieletsel via een mechanisme dat afhankelijk is van de opening van mitochondriale K(ATP)-kanalen.
