Cilostazol is een krachtige cyclische nucleotide fosfodiësterase type 3 (PDE3) remmer met een IC50 van 0,2 μM en remmer van adenosine-opname.

In vitro

Cilostazol (OPC-13013) is een 2-oxo-chinolinederivaat met antitrombotische, vaatverwijdende, antimitogene en cardiotone eigenschappen. De vaatverwijdende en antiplaatjeswerking van deze verbinding zijn voornamelijk te danken aan de remming van fosfodiësterase 3 (PDE3) en de daaropvolgende verhoging van intracellulaire cAMP-niveaus. Het remt de bloedplaatjesaggregatie. Deze verbinding bezit ook het vermogen om de opname van adenosine te remmen. Het is aangetoond dat de verhoging van interstitiële adenosine door deze chemische stof in het hart de door de PDE3-remmende werking van Cilostazol veroorzaakte toenames in cAMP vermindert, waardoor de cardiotone effecten worden afgezwakt. Het wordt gerapporteerd de proliferatie van gladde spiercellen te remmen. Dit middel ontspant vasculaire gladde spieren en veroorzaakt vasodilatatie. Het remt de cytokine-geïnduceerde expressie van monocyt chemoattractant protein-1 (MCP-1). Deze verbinding verminderde plasma-triglyceriden en verhoogde plasma-HDL-cholesterol.
